Story Collection on Guardianship/ Conservatorship and People with Intellectual and Developmental Disabilities

Quality Trust for Individuals with Disabilities, the Arc of the United States, and the Autistic Self Advocacy Network are working together to gather stories for a report on adult guardianship and alternatives for people with intellectual and developmental disabilities. The report is being written for the National Council on Disability. Once the National Council on Disability approves it, the report will be publicly published and distributed on-line and in written form. 

By completing the below form, you are stating that you are age 18 years or older.  You also are agreeing that we can include the information you share in the report. Please feel free to write as much or as little as you would like in response to the questions we have asked. There is no right or wrong way to complete this form. You do not have to fill out this form or share your story if you do not want to. You may stop filling out this form at any time, without anything bad happening. We may edit the information you provide for length and clarity, and we will make sure not to use your name or link personally identifiable information to your story without your specific permission.
