Photo Release

I grant to the St Paul Figure Skating Club, its representatives and employees the right to take photographs of me and my property in connection to the above identified subject. I authorize the St. Paul Figure Skating Club, its assigns and transferees to copyright, use and publish the same in print and/or electronically. I agree that the St Paul Figure Skating Club may use such photographs of me without my name and for any lawful purpose, including for example such purposes as purposes as publicity, illustration, advertising, and Web content. I also understand that no royalty, fee or other compensation shall become payable to me by reason of such use. *

Parent/Guardian Consent

I understand that participation in this activity is completely voluntary and that the activity is being offered for the benefit of the participant. I agree that the St. Paul FSC, as well as any and all employees, shall not be liable for any claims, injuries, damages, of whatever nature, incurred by the participant during the activity. I expressly release and discharge St. Paul FSC and all agents or employees, from any such claims, injuries, or damages. *
