St. Paul FSC 2020-2021 Coach Membership Form 
June 1, 2020-May 31, 2021

Home Club Membership or renewal is granted after receipt of membership application, membership fee. USFS testing and competition privileges commence upon submission of Home Club application form and Home Club membership fee.

COACH COMPLIANCE

U.S. Figure Skating appreciates the cooperation and understanding of all members to help implement these minimum requirements to ensure that the environment in all U.S. Figure Skating programs is as safe as possible for all participants. This will help give parents the highest degree of confidence that their children are not only receiving good care and proper instruction but are safe from improper conduct.

Six Requirements
1. U.S. Figure Skating Full Membership (either through a member club or as an individual)
2. Completion of SafeSport Training
3. Successfully pass the annual background screen
4. Verification of current coach liability insurance
5. Completion of CER Courses
6. Additional requirement - PSA Membership if coaching in qualifying competitions only

Note: Non-compliant coaches who attempt to coach at a test session or competition are subject to disciplinary action by U.S. Figure Skating and/or the PSA. *

HOME CLUB STAFF COACHES:  
Coaches who teach at Pleasant Arena or our affiliates (Highland LTS, Richfield LTS and Coon Rapids Jr Club).  Staff Coaches are required to teach one number or the equivalent in Richfield Ice Show and teach in the St. Paul FSC Basic Skills Programs.
Membership includes US Figure Skating Membership & Skating Magazine, and voting privileges.  Contact Skating Director Ann Eidson regarding teaching privileges.
HOME CLUB GUEST COACHES:
Coaches who teach at St. Paul FSC and other rinks or clubs not affiliated with St. Paul FSC and coaches who are visiting from out of town. Membership includes US Figures Skating Membership & Skating Magazine, and voting priviledges. Contact Skating Director Ann Eidson regarding teaching privileges
ASSOCIATE GUEST COACHES:
Coaches who teach at St. Paul FSC, and other rinks or clubs not affiliated with St. Paul FSC as well as coaches who are visiting from out of town. These coaches have home club membership status at another USFS club.
Medical Emergency Reference Form
Each skater is required to print, compete and return the 2020/21 Medical Emergency Form posted on the website. 
Please mail  your form to SPFSC c/o Loreen Fripp 14655 Waco St NW, Ramsey MN 55303 or place in the Club Locker at Pleasant Arena in an envelope.
It is member's responsibility to keep the emergency information updated throughout the year.

WAIVER & RELEASE OF LIABILITY,  ASSUMPTION OF RISK & INDEMNITY AGREEMENT
In consideration of participating in St. Paul Figure Skating Club activities, I represent that I understand the nature of figure skating activities (“activity”) and that I am qualified, in good health and in proper physical condition to participate in such “activity”.  I acknowledge that if I believe event conditions are unsafe, I will immediately discontinue participation in the “activity”.

 

I fully understand that this “activity” involves risks of serious bodily injury, including permanent disability, paralysis and death, which may be caused by my own actions, or inactions, those of others participating in the “activity”, the conditions in which “activity” takes place, or the negligence of the “releasees” named below;  and that there may be other risks either not known to me or not readily foreseeable at this time; and I fully accept and assume all such risks and all responsibility for losses, costs, and damages I incur as a result of my participation in the “activity”.

 

 I hereby release, discharge, and covenant not to sue the St. Paul Figure Skating Club,  United States Figure Skating, it’s directors, officers, administrators, sponsors, volunteers, agents, employees, staff, instructors, trainers, other participants and if applicable, owners and lessors of premises on which the “activity” takes place (each considered one of the “Releasees” herein) from all liability, claims, demands, losses, or damages on my account caused or alleged to be caused in whole or in part by the negligence of the “releasees” or otherwise, including negligent rescue operations;  and I further agree that if, despite this release, waiver of liability, and assumption of risk, I, or anyone on my behalf, makes a claim against any of the Releasees, I will indemnify, save, and hold harmless each of the releasees from any loss, liability, damage or cost which any may incur as the result of such claim.

 

The St. Paul Figure Skating Club has the right but not the obligation, to provide rules, regulations and/or ice monitors for Club Ice.  We hereby acknowledge the St. Paul Figure Skating Club shall not be responsible for the supervision of the members at Club Ice.

 

I have read this RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT, understand that I have given up substantial rights by signing it and have signed it freely and without any inducement or assurance of any nature and intend it to be a complete and unconditional release of all liability to the greatest extent allowed by law and agree that if any portion of this agreement is held to be invalid, the balance, notwithstanding, shall continue in full force and effect.

I agree to abide by, and to conduct myself in accordance with, the By Laws and official rules of U.S. Figure Skating and St. Paul FSC, and I agree to abide by such other rules, policies and procedures as may be approved, in accordance with the By Laws, by the St. Paul FSC Board of Directors. *

e pictures of me taken during the membership year 6/1/2020-5/31/2020 in any ads and/or media releases, now and in the future, that they deem appropriate to further public awareness of the St. Paul FSC program and the sport of figure skating.
