St. Paul FSC 2017-2018 Membership Application
June 1, 2017-May 31, 2018

All persons wishing to become a member of St. Paul FSC must submit a club membership application form and submit payment for the membership dues for the current season. Dues are to be determined by the Board of Directors as deemed reasonable to insure the financial solvency of the Club.

Home Club and Associate Membership or renewal is granted after receipt of membership application, membership dues, and upon a majority vote of the Board of Directors. Ice credit package Purchase Privileges will commence upon receipt of the membership application and membership dues. Voting privileges will commence upon application approval by Board of Directors.

SKATER INFORMATION
MEMBERSHIP VOLUNTEER REQUIREMENT

This year the St Paul FSC is hosting competitions:

52nd Annual Skate St Paul (July 14-16, 2017 ~ Richfield Arena)

VOLUNTEERING REQUIREMENTS
All Home Club members are required to fulfill volunteering hours, the number of which shall be determined by the Board of Directors and reported to members for the following membership year prior to the membership renewal deadline.

2017-2018 VOLUNTEERING REQUIREMENTS
All Junior and Senior Club Members are required to volunteer a minimum of 30 hours per family during the 2017-2018 membership year. (However, this requirement is reduced to 15 hours if the skater participates in one of the SPFSC Competitions, as described below.)

All Introductory Home Club Members are required to volunteer a minimum of 15 hours per family during the 2017-2018 membership year.

SKATE ST. PAUL AND JUNE BUG COMPETITIONS
Skate St. Paul and June Bug are the club’s annual competitions and are the primary fundraising tools of SPFSC. Due to our particular type of 501 (c)(3) registration, the club is prohibited from traditional types of fundraising, and relies solely on these competitions and our annual ice shows for fundraising. All club skaters are expected to participate in Skate St. Paul or June Bug. Skaters who compete at these events will receive a credit of 15 volunteer hours for Skate St. Paul and 10 volunteer hours for June Bug (skaters who compete in both events will receive a credit of 15 hours). Credit for June Bug is available to Junior Club members only. Introductory Home Club members do not receive credit for competition participation toward their volunteer  requirement, because they have a lower minimum requirement of volunteer hours.

To fulfill this requirement for the 2017-2018 membership year, skaters must register in the 2017 Skate St. Paul event (held June 15-16, 2017 – Registration deadline June 26) or the 2018 June Bug held in June of 2018. Optionally, families may pay a fee equivalent to the entry fee of each event ($120 for Skate St. Paul, June Bug fee will be announced Spring 2018 – the fee for 2017 was $70) to receive the volunteer hour credit.

VOLUNTEERING BUY OUT OPTION
Families may opt out of any and all volunteering requirements by paying a $300 fee.

VOLUNTEER TRACKING AND FULFILLMENT VERIFICATION
Volunteer hours will be tracked by the SPFSC Volunteer Coordinator. At the end of the membership year each family’s volunteer hours will be assessed for fulfillment verification. Families who have not met the minimum number of hours required will be assessed a $10 per hour fee for every hour below the minimum number of hours required.

Note that a Senior Club membership is necessary to take US Figureskating tests higher than Pre Preliminary level.  Junior Club membership is necessary to take Pre Preliminary tests.

Medical Emergency Reference Form
Each skater is required to print, compete and return the 2015 Medical Emergency Form posted on the website. 
Mail to SPFSC 2185 Webber Hills Rd, Wayzata MN   55391, or place in the Club Locker at Pleasant Arena. 
It is member's responsibility to keep the emergency information updated throughout the year.
WAIVER & RELEASE OF LIABILITY,  ASSUMPTION OF RISK & INDEMNITY AGREEMENT
In consideration of participating in St. Paul Figure Skating Club activities, I represent that I understand the nature of figure skating activities (“activity”) and that I am qualified, in good health and in proper physical condition to participate in such “activity”.  I acknowledge that if I believe event conditions are unsafe, I will immediately discontinue participation in the “activity”.

 

I fully understand that this “activity” involves risks of serious bodily injury, including permanent disability, paralysis and death, which may be caused by my own actions, or inactions, those of others participating in the “activity”, the conditions in which “activity” takes place, or the negligence of the “releasees” named below;  and that there may be other risks either not known to me or not readily foreseeable at this time; and I fully accept and assume all such risks and all responsibility for losses, costs, and damages I incur as a result of my participation in the “activity”.

 

 I hereby release, discharge, and covenant not to sue the St. Paul Figure Skating Club,  United States Figure Skating, it’s directors, officers, administrators, sponsors, volunteers, agents, employees, staff, instructors, trainers, other participants and if applicable, owners and lessors of premises on which the “activity” takes place (each considered one of the “Releasees” herein) from all liability, claims, demands, losses, or damages on my account caused or alleged to be caused in whole or in part by the negligence of the “releasees” or otherwise, including negligent rescue operations;  and I further agree that if, despite this release, waiver of liability, and assumption of risk, I, or anyone on my behalf, makes a claim against any of the Releasees, I will indemnify, save, and hold harmless each of the releasees from any loss, liability, damage or cost which any may incur as the result of such claim.

 

The St. Paul Figure Skating Club has the right but not the obligation, to provide rules, regulations and/or ice monitors for Club Ice.  We hereby acknowledge the St. Paul Figure Skating Club shall not be responsible for the supervision of the members at Club Ice.

 

I have read this RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT, understand that I have given up substantial rights by signing it and have signed it freely and without any inducement or assurance of any nature and intend it to be a complete and unconditional release of all liability to the greatest extent allowed by law and agree that if any portion of this agreement is held to be invalid, the balance, notwithstanding, shall continue in full force and effect.

PARENTAL CONSENT AND INDEMNIFICATION AGREEMENT
I, the minor’s parent and/or legal guardian, understand the nature of the above referenced activities and the minor’s experience and capabilities and believe the minor to be qualified to participate in such “activity”.  I hereby release, discharge, covenant not to sue and AGREE TO INDEMNIFY AND SAVE AND HOLD HARMLESS each of the Releasees from all liability, claims, demands, losses, or damages on the minor’s account caused or alleged to have been caused in whole or in part by the negligence of the Releasees or otherwise, including negligent rescue operations, and further agree that if despite this release, I, the minor, or anyone on the minor’s behalf makes a claims against any of the above Releasees, I WILL INDEMNIFY, SAVE AND HOLD HARMLESS EACH OF THE Releasees from any litigation expenses, attorney fees, loss liability, damage, or cost any Releasees may incur as the result of any such claim

I agree to abide by, and to conduct myself in accordance with, the By Laws and official rules of U.S. Figure Skating and St. Paul FSC, and I agree to abide by such other rules, policies and procedures as may be approved, in accordance with the By Laws, by the St. Paul FSC Board of Directors. *
SPFSC CODE OF CONDUCT

The St. Paul Figure Skating Club supports a zero-tolerance policy for unsportsmanlike behavior. Its members including skaters, coaches, officials, parents and supporters should continually raise the standard of behavior both on and off the ice. Membership in SPFSC is a privilege and should carry a sense of pride. Participation in programs offered will require that all members accept certain responsibilities.

MEMBER/COACH RESPONSIBILITIES:

  • I will not verbally or physically abuse any of my fellow members, other USFS members, USFS officials, Club Board Members, volunteers, or coaches.
  • I understand that poor sportsmanship, on or off the ice, will not be tolerated.
  • I will continually strive to maintain a positive environment at SPFSC.  I will abide by this Code of Conduct Policy as provided by SPFSC.
  • I will abide by USFS Code of Conduct and Code of Ethics at all times.

PARENT/GUARDIAN RESPONSIBILITIES:

  • I will model and encourage sportsmanship by showing positive support for all members, coaches, board members, club officers, and other officials during all on and off ice sessions.
  • I will encourage my child to treat all other members, coaches, officials, board members, club officers and other member’s parents with respect.
  • I will refrain from coaching my child or directing other skaters during any on- and off-ice sessions (excluding parents who are staff or guest coaches at St. Paul FSC).
  • I will not verbally, mentally, or physically abuse any club members (including my own child), coaches, club officers, officials, or other club volunteers.
  • I will represent myself positively and not undermine the authority of coaches, club board members or officers, officials or other club volunteers.
  • I will also read and follow the Bylaws of SPFSC, and all other club policies and directives.

COURSE OF ACTION:

Should a parent or member not adhere to the above Code of Conduct, SPFSC holds the member, coach, and/or parent accountable and the Board of Directors is empowered to take necessary action. Actions could be as simple as a verbal or written warning up to suspension or expulsion from SPFSC. All decisions of the Board of Directors are final and binding. *

                         ST. PAUL FIGURE SKATING CLUB PHOTO RELEASE
The St. Paul FSC has my permission to use pictures of my child taken during the membership year 6/1/2015-5/31/2016 in any ads and/or media releases, now and in the future, that they deem appropriate to further public awareness of the St. Paul FSC program and the sport of figure skating.
