Criminal History Questionnaire

The AmeriCorps application process requires a criminal history check. This background check entails a search of the National Sex Offenders Public Registry, statewide criminal history repository check(s), and an FBI criminal history check, which requires fingerprints. The review process is not lengthy, and normally is completed within weeks.

You will not be permitted to serve or work with children, individuals with disabilities, or individuals over 60 years of age, without accompaniment, until the history check is complete and you are cleared.

Please answer the following questions fully. Existence of a criminal conviction or juvenile adjudication may or may not, depending on circumstances, disqualify you from consideration. However, any intentional misrepresentation or omission is grounds for disqualification. If you are unsure if something in your history is applicable, it is recommended that you list it, even if it is not disqualifying.

We do not complete the statewide criminal history repository check(s) and FBI criminal history check until after we’ve made a provisional offer.

Please answer the following questions fully.

Responses should be inclusive of misdemeanors and any offense that led to pre-trial intervention and/or fines other than speeding or parking tickets.

Have you ever been convicted as an adult, or adjudicated as a juvenile offender, of at least one criminal offense by either a civilian or military court? *
Are you currently facing charges for any offense, on probation, or on parole? *
Are you registered or required to register on a sex offender registry? *

Criminal History Verification:

You have indicated that you have not been convicted of a criminal offense and are not currently facing charges. Any intentional misrepresentation or omission will disqualify you from service. The program will make the determination as to whether any misrepresentation was intentional.
