Please complete this form by providing the names of your student chapter leaders and other pertinent information.  Please use full names, addresses and formal titles (e.g., John Doe, Ph.D., SPHR, Professor) and complete the form as completely as possible.  Notify SHRM immediately of any changes in your chaper information.
Submission of this completed form by June 1 of each year is a requirement to maintain your student chapter's active affiliation with SHRM for the upcoming academic year and eliibility to participate in the Student Chapter Merit Award Program.
