MEMBERSHIP CHANGE, FREEZE AND CANCELLATION REQUEST FORM - Front Range Fitness for Women

You are welcome to change the type of your membership anytime. For an unlimited membership, the Change in Membership Request requires a 30 day notice prior to your billing date. If your request is less than 30 days before your scheduled billing date, your credit card will be charged one more time on the scheduled billing date. Change in membership requests require a 30 day notice prior to your expiration date for a punch card membership.

If you are a member who has an unlimited, auto withdrawal membership you are welcome to freeze your membership for up to 30 days, consecutively or non- consecutively. Freezing requests require a 30 day notice prior to your billing date. If your request is less than 30 days before your scheduled billing date, your credit card will be charged on the scheduled billing date and not charged on the billing date following.

I understand that I can freeze my membership if submitted in writing at least 30 days prior to my next billing date. If my request falls less than 30 days before my billing date, my credit card will be charged on the scheduled billing date and not charged on the billing date following.

I understand my payment will automatically restart upon the predetermined date, unless I notify Front Range Fitness for Women for an earlier reactivation date. I agree my card will begin to be charged on that date.

I understand that if my freeze request is for a time period less than 30 days, my expiration date will be extended by the same number of days that my membership was frozen.

Membership Freeze Policy: Circumstance beyond your control (loss of job, illness/injury, family situation):
If you are experiencing a situation beyond your control, you may freeze your membership for up to 30 days; at the end of this time period we will re-assess your situation and make adjustments to your membership as mutually agreed upon. Your expiration date will be extended the total number of days you are gone. (up to 30 days).

I understand that I can cancel my membership if submitted in writing at least 30 days prior to my next billing/expiration date. If I have an unlimited monthly membership and my request falls less than 30 days before my billing date, my credit card will be charged one more time on the scheduled billing date.

Front Range Fitness for Women will not refund your investment fee. You have made a valuable investment in your health and your future, and once your investment is made, Front Range Boot Camp takes on the responsibility to support you in your journey. We are serious about you, and by investing in Front Range Boot Camp, you are committing to being serious about yourself as well.

Memberships are not transferable.
