Use of this form to request an increase to your current card limit does not guarantee acceptance of the proposed increase.  The granting or extension of credit card limits is made by the Credit Committee and/or Loan Officer of this credit union.  You will be notified in writing of that decision and provide written explanation in the event the credit is declined.  You have the right to make a written appeal of that decision.

Additional Credit Line Disclosures

FEDERAL LAW PROHIBITS THIS INSTITUTION FROM RAISING YOUR CREDIT LIMIT WITHOUT YOUR EXPRESS WRITTEN PERMISSION.

If we approve your application for a credit increase, we will establish a new self-replenishing Line of Credit for you and notify you of its amount.  Continue to use the card issued to you.  You agree not to let the account balance exceed this approved Credit Line.

Each payment you make on the account will restore your Credit Line by the amount of the payment which is applied to principal.  You may request another increase in your Credit Line, but only by new written application to us, which must be approved by our credit committee or loan officer.

Further, by giving you written notice our credit committee may reduce your Credit Line from time to time, or with good cause, revoke your card and terminate this Agreement, or our adverse reevaluation of your creditworthiness.

Regarding your rate and rate increases, unless the rate increase was disclosed to you when you first opened the credit card, there can be no interest rate increases during the first 12 months of opening the card.  Promotional rates will be, by law, at least 6 months in duration.  In the event of rate increase, we shall not increase rates on pre-exisiting balances unless you become more than 60 days late on your credit card payments.  We must give a 45-day advance notice before increasing your interest rate or making any major change to your credit card agreement.  Your interest rate could increase if you don't make the minimum payment within 30 days of your due date, even during the first 12 months of opening your account.  You must recieve a 45-day advanced notice of penalty rate increases.  Increased rates must be reviewed by us and lowered if the review shows improved payment habits.

You may also terminate this Agreement at any time, but termination by either of us does not affect your obligation to pay the account balance.  The cards remain the property of the Credit Union and you must recover and surrender to us all cards upon our request and upon termination of this Agreement.

You hereby agree that the credit union may keep the portion of the Agreement which bears your signature so we can comply with federal and/or state law relating to loan documents the credit union is required to keep in its file.

I/We authorize the Credit Union to verify information provided above and to request a credit report if necessary.  Further, in accordance with Regulation V, the Fact Act, we may report information about your account to the credit bureaus.  Late payments, missed payments, or other defaults on your account may be reflected in your credit report.
