Two (2) copies of the employee network files (h drive on CD's) will be provided to the Supervisor listed above and removed from the SRC network immediately following the employees last day at work.
Did this employee have an e-mail account?
If yes, what would you like to do with the employee's incoming e-mail?
If forwarded, for how many days?
Did the employee have an office telephone?
All employee email and telephone accounts are automatically forwarded to the Supervisor listed above
unless the Supervisor indicates the email and/or telephone extension of a temporary replacement employee here:
Emails are not forwarded to the former employee.
All email accounts kept active will contain the following automatic reply message: "The person with this email address is no longer an employee of Spoon River College. Please call 309-647-4645 or email
to contact the appropriate person"

You are done filling out the check out form. Make sure the box is selected for network/email and then scroll down to submit the form to IT!
