Cole Academy Space-Use Request Form

Thank you for your interest in co-locating at one of our campuses. We currently have limited special-services space available at many of our schools, plus for 2015 we have open availability at our former Kunia site, our two-level, 7,500 square foot space at KUNIA SHOPPING CENTER, 94-673 Kupuohi Street.

 

To begin the process of arranging space for delivering your services, please fill out the following request form. A representative will be back in touch if a match can be made. Space reservations are not confirmed automatically on this form. We will make a best effort to accommodate requested time-slots but final assignments are based on availability, our approval and your approval, and upon proof of adequate insurance and additional-insured certificates.

Please indicate the timeslot(s) you wish to reserve, a short description of the services, and your max-capacity for attendance. We will try to apportion the space based on the best-available fit for your service.

* TIMESLOTS ARE 90 minutes, inclusive of set-up and break-down/cleanup.

* EQUIPMENT AND SUPPLIES to be self-provided

* WAIVERS: Liability waivers must be signed by all providers and customers prior to space use.

* COST OF RENTAL is 20% of retail/gross revenue from services during each timeslot, minimum $25 per 90-minute timeslot. Payment is due daily based on final headcount and revenue, and will be processed on a credit- or debit-card to be placed on file with our secure credit-card processing gateway.
