By signing this reservation form, I agree to pay a $15 transportation fee. If for some reason I wish to cancel transportation, I understand I must contact studentlife@highlandcc.edu at least one week in advance of transportation date, otherwise I forfeit my $15 fee.

Highland Community College reserves the right to cancel this transportation for any reason, including inclement weather. In the case that HCC cancels the transportation, all students will have a full refund of transportation fee to their account. Students will be notified through the email that they sign up for transportation with. HCC will make every effort to notify students as soon as possible. 

PAYMENT OPTIONS:

You may pay using a debit/credit card, cash, check or Cash App. Payment is due PRIOR to using transportation. For the dates listed above, you must have paid for your ride by Wednesday, November 18th, at 5 p.m. Visit the Student Life office in the MSCT Building or the Residential Life office in the Walters Wellness Center to pay.

The undersigned students, employees, and invitees of Highland Community College, being 18 years of age or older, acknowledge that they have signed up for above named transportation, and that this service is voluntary on their part. The undersigned therefore assume all risk incident to the above listed transportation, including travel to or from such place of service and acknowledge that Highland Community College, its Board of Trustees, its employees and the State of Kansas assume no liability, including financial responsibility for injuries or losses resulting from or occurring during transportation. The undersigned further indemnify, release, and hold harmless Highland Community College, their Board of Trustees, their employees, and the State of Kansas against any loss or injuries resulting or claimed to result from the undersigned students’/employees’ training or other involvement in any and all phases of transportation. The undersigned further acknowledge that their signatures hereto evidence that they have read, fully understand, agree with and consent to the contents thereof. IF UNDER 18, A STUDENT’S PARENT OR LEGAL GUARDIAN MUST SIGN BELOW.
