Any questions our representatives have about an item being donated will be directed to the person whose name appears on this registration form.

  • Registrations will be limited to four per decorator. Each centerpiece must be unique from any others submitted by the same decorator. 
  • Please submit a photo of your item with your registration. If a photo is not currently available, please submit a photo by October 15.
