• Please submit a "Return Authorization Request Form" within by January 14, 2012.

• We will send you a Return Authorization Number within 4 days from the day you submit your request.

• Once you receive your RA#, please mail your item back to us within 10 days from the day you receive the RA to:

• Your return/exchange will be processed within 2 weeks from receipt
