Remember that our league is a non-contact recreational league. Unsportsmanlike play will NOT be tolerated and will be dealt with up to and including suspension from the league. Registration is on a first paid, first serve basis. ALL FEES MUST BE PAID IN FULL BEFORE THE START OF THE 1ST GAME OF THE SEASON. NO EXCEPTIONS. A 50% Refund will be only be given for injury or pregnancy up to the 3rd week of play (a medical note is required). COST: Goalies $75.00 Players $150.00.

WAIVER

I hereby agree to indemnify and save harmless DONNA BRENNAN, representative of the Brampton Ladies Ball hockey League, from any and all claims that may arise during the 2008-2009 winter season and that I hereby waive my right to name DONNA BRENNAN legally responsible for any injury or problem that arises during this season.
I understand that I am playing in a adult rec. league and that I hold myself solely responsible for anything that happens to me during this season.
I acknowledge that I have read and understand this application to play in the Brampton Ladies Ball Hockey League 2008-2009 winter season.
By submitting my registration for the 2008-2009 winter season, I agree to the above.
