2009 ITCA Triathlon Volunteer Registration Form

Thank you for volunteering for the 13th Annual ITCA Triathlon presented by Atlantic Sports Health to benefit Special Olympics New Jersey Area 3 (serving Morris, Sussex & Warren Counties). Please fill out the form below and indicate your areas of interest.
You will be contacted by your Team Captain once assignments are finalized. Team Captains will not be able to confirm volunteer roles until late August/early September.
*Certain volunteer roles take place before event day (Saturday, September 26th) please choose in accordance with your schedule availability.
PLEASE NOTE:
Volunteers MUST be at least 15 years old on event day
to participate. Many of our roles require physical strength or time spent in the wooded areas of Stephens State Park. While we appreciate all inquiries for the safety of our volunteers and our athletes this is a mandatory requirement.

WAIVER, RELEASE & INDEMNIFICATION

Please read carefully before signing. In consideration of my acceptance to the 2008 ITC Triathlon ("the event"): I hereby agree to comply with all the rules, regulations and instructions of the event and its directors. For myself, my executors, administrators, heirs, next of kin, and assigns, I hereby: waive and release any and all claims that I may have against the event, its event committee, any officers, directors, members, volunteers, employees, agents, sponsors, the Township of Mount Olive, the International Trade Center, the International Trade Center Alliance, NJ Foreign Trade Zone Venture, LLC, 400 ID Partners, JP Morgan Inc., CB Richard Ellis Real Estate Services Inc., CBRE, Rockefeller Development Corporation, Mount Olive Industrial Realty Company, LLC, 550 Clark,LLC, 850 Clark, LLC, Atlantic Sports Health and all its affiliates, Atlantic Health and all its affiliates, GreenBrook Racing LLC, Pavillion Beach, Kittatiny Canoes, Event sponsors, their officers, agents and employees, or any of their executors, administrators ("the releases"), any and all claims for damage caused by the negligence of any of them, arising out of my participation in the event and its related activities, togehter with any costs including attorney's fees incurred as a result of such claim whether valid or not, and Indemnify and hold harmless the releases and each of them against any such claim that I or my gusts or any one of my executors, administrators, heirs, next of kin, successors or assigns may have or assert against any costs including attorney's fees with respect there to. I hereby acknowledge that I have sole responsibility for my possessions and athletic equipment before, during and after the event and its related activities. I hereby acknowledge that hte event is extremely strenuous, difficult and hazardous even under the most favorable conditions. I understand my participation as a volunteer carries risk of personal injury, sickness and/or death including but not limited to those caused by terrain, my physical condition, vehicular and pedestrian traffic and other participants in the event. I understand and I hereby accept these and all other risks and hazards inherent in this event and its related activities. I hereby attest that I am physically fit and have sufficiently trained for this event and that my physical condition has recently been verified by a licensed medical doctor. I hereby consent to receive medical treatment which may be deemed advisable in the event of injury, accident and/or illness during the event. I hereby permit the free use of my name and pictures in broadcasts, telecasts and the press as they pertain to the event. I understand that parts of the course of the event will be open public roads and highways that will not be closed to the vehicular and pedestrian traffic. I fully accept the risks and hazards that this implies, including collisions and falls during participation and I assume complete responsibility of my own actions on all highways, roads, paths and sidewalks that comprise the course. I further certify that I have read and fully understand tha above Waiver, Release and Indemnification Form and the contents were completely and candidly understood by me and that I am voluntarily consenting to:
