Remember that our league is a non-contact recreational league. Unsportsmanlike play will NOT be tolerated and will be dealt with up to and including suspension from the league. Registration is on a first paid, first serve basis. ALL FEES MUST BE PAID IN FULL BEFORE THE START OF THE 1ST GAME OF THE SEASON. NO EXCEPTIONS. A 50% Refund will be only be given for injury or pregnancy up to the 3rd week of play (a medical note is required). COST: Goalies $75.00 Players $170.00

WAIVER

I hereby agree to indemnify and save harmless DONNA BRENNAN, representative of the Brampton Ladies Ball hockey League, from any and all claims that may arise during the 2018 summer season and that I hereby waive my right to name DONNA BRENNAN legally responsible for any injury or problem that arises during this season.
I understand that I am playing in a adult rec. league and that I hold myself solely responsible for anything that happens to me during this season.
I acknowledge that I have read and understand this application to play in the Brampton Ladies Ball Hockey League 2017 summer season.
By submitting my registration for the 2018 summer season, I agree to the above.
